What Is Cardiopulmonary Exercise Testing?

A cardiopulmonary exercise test (CPET) is a non-invasive gold standard assessment of aerobic fitness. CPET assesses exercise capacity by looking at how an individual’s heart and lungs are able to cope with the increasing demand of incremental exercise.

Why is Cardiopulmonary Exercise Testing Performed?

CPET is used in the assessment of a number of clinical conditions and has been an established clinical service in Harrogate since 2011. Clinical applications include:

  • Pre-operative assessment of fitness prior to major surgery
  • Evaluation of patients with cardiovascular disease
  • Investigation of unexplained breathlessness
  • Investigation of exercise intolerance

The CPET is also used extensively for athletes and in sports training programmes.

How is the Cardio Pulmonary Exercise Test conducted?

The test is conducted on an exercise bike with the heart rate, blood pressure and oxygen saturations continuously monitored. A facemask measures lung volumes and inspired and expired respiratory gases are continuously collected. The individual then cycles against increasing resistance for as long as they can, until exhaustion. The test usually lasts between 8-15 minutes. The data is then analysed to build a picture of the individuals cardiovascular fitness and limiting factors to exercise.

Cardiopulmonary Exercise Testing for athletes

Most elite athletes, including many of our Olympians, use CPET as part of the assessment of the effectiveness of their bespoke training programs. CPET can identify two key performance metrics: the VO2 Max, a marker of ‘fitness’ and the anaerobic threshold, an important metric for endurance training and identifying accurate heart rate training zones. Despite the proven benefit of CPET in guiding physical training, there has been extremely limited access for amateur athletes. We aim to change this by offer individual CPET testing at affordable prices.
